Anyone in the United States eligible for adjustment of immigration status must submit form I-485, accompanied by form I-693, the immigration medical exam. Patients must bring a valid government-issued ID, such as a driver’s license or passport. Student or work IDs are not accepted. Children under 14 must bring a birth certificate. IDs must be presented at the initial visit; photos or photocopies are not accepted. If you have an Alien number and/or a USCIS online account number, this information must be included on form I-693.

The Exam Process: What to Expect

Generally, the process requires two visits:

First Visit

During your first appointment, we will complete the full physical examination, collect blood and urine samples, review your vaccination records, and administer any required vaccines. This visit typically takes 30 to 45 mins. Payment in full is required at the time of the first visit.

Second Visit

At your second visit, Dr. Melendez will review your laboratory results and finalize Form I-693. You will receive a sealed and signed envelope that you cannot open. This second visit is usually scheduled about a week after the first.

*Additional visits may be necessary if any lab results require follow-up.
