Anyone in the United States eligible for adjustment of immigration status must submit form I-485, accompanied by form I-693, the immigration medical exam. Patients must bring a valid government-issued ID, such as a driver’s license or passport. Student or work IDs are not accepted. Children under 14 must bring a birth certificate. IDs must be presented at the initial visit; photos or photocopies are not accepted. If you have an Alien number and/or a USCIS online account number, this information must be included on form I-693.

Exam Process

Generally, the process requires two visits:

First Visit

Complete medical history, physical examination, laboratory tests, and vaccination review.

Second Visit

Review documentation and discuss lab results. You will receive a sealed and signed envelope that you cannot open. This second visit is usually scheduled about a week after the first.

*Additional visits may be necessary if any lab results require follow-up.
