Urgent Care Visits

Include complete medical evaluation, physical examination, basic labs, medications, and infusions at the medical facility

Annual Physical Exam

Includes comprehensive medical evaluation, physical exam, health risk assessment, and comprehensive labs in-house. EKG included in all annual physicals as necessary for members over 40 years of age.

Consider your medical needs. If you frequently need medical care, consider a membership with more comprehensive coverage. If you seek coverage mainly for urgent care services, a more economical plan may suffice.
02How and when can I enroll in a membership? What is the enrollment process, and is there a specific enrollment period?

You can start your membership at any time. There is no specific enrollment period nor an application process. Simply fill out a service agreement with the clinic based on your choice.

03How do you handle pre-existing conditions? Are there any restrictions or waiting periods for pre-existing conditions?
There are no restrictions or waiting periods for pre-existing conditions. Pre-existing conditions are evaluated during the first visit, and appropriate care is established based on the patient’s preferences and the doctor’s recommendation.
04What support options and resources are available for members? Do you offer telephone medical advice, mobile apps, or case management?

We provide telephone outreach, email, and text messaging for quick communication with the doctor or clinic staff. Messages and calls are answered within 24 hours. We also offer a patient portal where you can access your documents at any time.

05How can I make changes to my medical plan once enrolled? What changes are allowed, and how can I request them?
Changes can be made at any time without additional charge. You should contact the staff to arrange the changes. Note: Only cancellations incur a fee.
